I have mainly worked on the effects of hypertension on brain ageing and on vascular brain lesions on cerebral MRI. I was first author of a landmark paper reporting on a study conducted as part of an international randomised trial to lower blood pressure, showing that it is possible to reduce the risk of post- stroke dementia (Ref 10).

With my team and using data from the 3C study, a population-based cohort study in the elderly, we were the first to show the association between blood pressure variability and dementia (Ref 9), suggesting new pathophysiological mechanisms of dementia. We have followed up this work and uncovered some important aspects of this association, both in terms of mechanisms and effects of antihypertensive treatment (Ref 5 and Ref 7). My expertise in this area has led me to write editorials for major hypertension and neurology journals and to participate in an American Heart Association working group (Gorelick et al. Stroke 2011, 2250 citations). I have been involved in several large international collaborations including a recent meta-analysis which is a Highy Cited Paper (Ref 4) and a study on vascular health and dementia (Ref 6).

In exploring the intermediate factors of the association between hypertension and dementia with cerebral MRIs in the 3C study, we were the first to show that enlarged perivascular spaces (ePVS) were associated with both hypertension and dementia risk (Ref 8), which was later confirmed in several studies. I am co-senior author of a recent paper (Ref 1) deciphering the molecular determinants of ePVS.

More recently, I have been working on the early determinants of brain ageing by studying cerebral MRIs of 1800 young adults (i-Share cohort; in progress) Because of the uniqueness of this sample in this age range we have been asked to participate in a large international brain mapping collaboration (Ref 1; Highly Cited Paper). Because of the covid epidemic, we have also been working on health-related topics such as vaccine acceptance in young adults (Ref 3; Highly Cited Paper and Hot Paper).
