Professor Wardlaw trained in medicine (1982) and specialised in neuroradiology (1992) with a particular clinical and research interest in causes, diagnoses, clinical consequences and treatments for cerebrovascular disease. \\
\\
From 1990-2014 she trialled thrombolytic therapies to treat acute ischaemic stroke, now used routinely to treat stroke  and alteplase is now a WHO essential drug. She also advanced use of imaging in stroke diagnosis and prevention. \\
\\
From 2000-date, she has focused on cerebral small vessel disease (SVD), a common cause of stroke and dementia, determining the pathology, vascular dysfunction, brain lesions, trialling drugs and improving clinical services. She initiated and led the LACunar Intervention Trials (LACI-1, LACI-2, ISRCTN14911850) testing feasibility of repurposed drugs to prevent stroke and dementia in patients with SVD. Based on promising results of LACI-2, she now leads the UK NIHR-funded phase 3 efficacy trial, LACI-3, to prevent dementia and stroke in 1300 patients with SVD. \\

She holds a UK Dementia Research Institute Foundation Chair, leads a Centre in SVD research, a BHF Research Excellence Award theme on cerebrovascular disease, the UK DRI-BHF Centre for Vascular Dementia Research, and a UK-wide study on vascular cognitive impairment. \\
\\
She pioneered in vivo human clinical, epidemiological and neuroimaging methods to detect cerebrovascular damage and dysfunctions linking to translational models and clinical interventions. \\
\\
She set up several MSc and PhD training programmes to teach brain imaging methods and causes, consequences and treatment of cerebrovascular diseases.\\
