* 2021(01) - present Professor of Medicine and Head of the Department of Internal Medicine, Radboud University Medical Center, Nijmegen, The Netherlands. Since 2023 (01): Research Domain Leader for the Radboudumc Research Institute for Medical Innovation.
* 2010 - 2020(12) Professor of Medicine, Thrombosis and Haemostasis, at the Departments of Internal and Vascular Medicine, Academic Medical Center, Amsterdam. Co-chair of the Department of Vascular Medicine (2010-2018). Principal Investigator of the AMC.
* 2006 - 2010 Associate professor at the Department of Clinical Epidemiology (0.5 fte), and Department of General Internal Medicine (0.5 fte), Leiden University Medical Center, Leiden.Project leader of the NEO Study (Netherlands Epidemiology of Obesity Study)

__Honours and Awards__
* 2021 Heijmans van den Bergh medal, a once-per-five year, most prestigious honorary distinction of the Netherlands Internal Medicine Society for her outstanding achievements in terms of science, education, governance and being a role model for young internists. Although this medal has been awarded 12 times before (since 1960), Professor Middeldorp was the first female internist on whom this  medal was bestowed
* 2016 plenary Ham-Wasserman Lecture and received the accompanying award in recognition of pioneering work in inherited thrombophilia at the annual American Society of Hematology (ASH) meeting in San Diego 
* 2012 VIDI innovative research grant from the Netherlands Organisation for Scientific Research (NWO) “Thrombophilia and reproduction: mechanisms and targeted interventions”
* 2008 Clinical Established Investigator of the Netherlands Heart Foundation “Unravelling the common etiology of venous thrombosis and arterial cardiovascular disease”
